ESTABLISHED AT 1999

Dr. Jacqueline Delmont is a Board Certified Internist who has built a remarkable career serving low-income and underprivileged patient populations across Long Island and Queens. Through her own efforts, dedication and the force of her personality, Dr. Delmont was able to help thousands of men, women and children obtain the medical care they need and dramatically improve their lives.

In 1999, Dr. Delmont established her private practice in the Freeport-Roosevelt area, known as a federally-designated healthcare professional shortage area. In 2005, Dr. Delmont became the Operator and Medical Director of a failing Diagnostic and Treatment Facility (Primary Healthcare Plus) in Franklin Square, transforming the organization into a thriving treatment facility, delivering efficient and high quality care to thousands of low-income, uninsured and underinsured patients.

In addition to owning and operating four primary care practices (three of which are NCQA Level 3 PCMH certified), Dr. Delmont also serves as the Chief Medical Officer for an Independent Physician Association and Accountable Care Organization, Beacon Health Partners. Her work in this organization is directed at increasing the quality of care and patient experience of the population at large while decreasing the cost and inefficiencies surrounding that care.

Through trial and error I have developed techniques and methods that can be applied in many settings. I helped scores of at-risk men, women and children obtain the medical care they needed and dramatically improve the quality of their health.
